Parental / Child Waivers
This waiver is for parents who wish to sign a waiver on behalf of their child. This waiver should be completed by the parents of children who are attending parties at Special Ops.

Youth Group Waiver
This waiver is for youth groups and children taking part in any of our activities where a parent signature is not possible. It must be printed and signed prior to any game taking place. This waiver is not required for adult groups. Adults must sign their own individual waiver.
